From a physician in Massachusetts, USA:

Are there any A1c Hgb or fructosamine monitors for the patient for home use in diabetes?

There are a few hemoglobin A1c kits which can be used to collect a sample at home and the sample is mailed to a lab: B-D A1c at home kit (Becton Dickinson), A1c Express Hemoglobin A1c (Express-Med, Inc), Accu-Base Hemoglobin A1c Sample Collection Kit (Diabetes Technologies, Inc), and DSS A1c Home Collection Kit (Diabetes Support Systems).

There is also the Duet Diabetes Control System (LXN Corporation). This is a home meter that will do blood glucose and fructosamine testing and give the result on the meter.
