Why is it that some people can have a 500 blood glucose, but not have ketones, while I can be at 240 or thereabouts and be spilling large ketones in my urine?

My 17 year old son who has been on a pump for a week, has had two episodes of extremely high glucose readings. The first resolved fairly easily even though he had large ketones in his urine, but he was very sick with vomiting and fatigue during this second one. What varying factors are there to explain the difference? Could my son still have been in DKA with very high blood sugar and only a trace of ketones?

Does a person with Type 1 diabetes always have sugar in their urine? My daughter, who has diabetes, had a routine urine test done and there was sugar but no ketones in the urine. Her last blood sugar was 196 about 1 hour prior to the urine test. She then had her shot and her dinner.
