If one is using an insulin pump with Lispro and the insulin supply is terminated, then how long does it take before ketoacidosis is a severe problem?

Probably in about 2 to 3 hours. Lispro insulin [Humalog� brand] only lasts a short while, and there is some delay because of the depot under the skin, but I wouldn’t want to have a lack of insulin for than about an hour.
Whenever there is lack of insulin delivery from the pump, you should check blood glucose levels about every 1 – 2 hours to monitor what’s going on. That way, you can supplement with an injection if you need to. You should always have a vial of short-acting insulin (Lispro or Regular) and a syringe with you.
