Question from a physician in Alabama, USA:

My 5 year old daughter has had Type 1 for 3 years. Recently she had routine blood work drawn which demonstrated that her TSH level was extremely elevated while her T4 and T3 levels were still within the normal range. Suspecting that she is developing hypothyroidism, we have started her on Synthroid [a brand of thyroid hormone replacement therapy].

What is the incidence of thyroid dysfunction in young children with Type 1 diabetes and what are some other endocrine abnormalities that we should watch out for, and their incidences?

We look for thyroid problems because they occur in at least 20% of kids with diabetes. Her thyroid problem was found early since the active numbers [T3 and T4] were normal. Treat it as directed and all will be fine.

Actually there are reports of other endocrine problems, adrenals, etc., but this is very rare. The thyroid is the main issue.
