Question from New York, New York, USA:

My daughter uses a pump, and we are going to be traveling to a time zone that is five hours behind where we live. We are planning to stay for 10 days.and we want some guidance on adjusting her basal rates. Any thoughts?

On an insulin pump, it’s all rather easy. There are many different ways to do this, and all of them work as long as you are paying attention to choices of food and covering food with appropriate bolusing. You should also look in Pumping Insulin by John Walsh, P.A., C.D.E., and Ruth Roberts, M.A. as well as the time travel section in Insulin-Dependent Diabetes in Children, Adolescents and Adults, by Dr. Ragnar Hanas.

We usually have folks travel without changing basal rates and then change pump time to the new time zone once they arrive. As long as you do some more blood glucose testing the first few days and make any minor adjustments to cover meals, you should do well with this protocol. The key is lots of blood glucose testing so that you are not guessing.

Have fun on your trip.
