Question from Egypt:

My son’s mother is a general practitioner. She discovered, by accident, that our son, who weighs 28 kg (62 pounds), has diabetes. His fasting blood sugar was 170 mg/dl [9.4 mmol/L] while his postprandial blood sugar was 220 mg/dl [12.2 mmol/L]. A consulting pediatrician determined that his A1c was 9.0 and put him on Glucophage and asked his mom to adjust the dosage as necessary. For now, he is taking one-half a 500 mg tablet after lunch and the same amount after dinner. His blood sugars are now in the normal range. A second pediatrician said that our son should be on insulin. What is your opinion?

It’s hard to really know. If the A1c is the same standards as in Europe/US, then his average glucose is quite high. It is hard to imagine managing a 10 year old who weighs 28 kg other than as type 1 or needing insulin.
