Hyperglycemia and DKA, Insulin

Question from Missouri, USA:

We have been checking our son’s two hour post meal numbers. They are anywhere from 200 to 500 mg/dl [11.1 to 27.8 mmol/L]. We count carbohydrates and adjust the insulin based on what he ate. Does this indicate we need more fast-acting insulin or that he would need more Lantus (24 hour) insulin?

It seems to me that your son might need either more fast-acting insulin at meals and/or possibly waiting more between the shot and the beginning of eating.
