Question from Runcorn, Cheshire, UK:

We are vacationing to San Diego soon from the United Kingdom. Any advice about long haul fights, time zones, etc. for our 12 year old daughter (IDDM for 9 years) would be gratefully received.

You should discuss your precise travel schedule with your team. What we usually recommend is splitting the total daily insulin dose into 4 equal doses of soluble insulin (for example, Actrapid or Humulin S) then giving this every 6hours with food. This method allows for the longer day travelling west and the shorter day coming home. Once you start your first full day in America you can revert to your usual pattern — perhaps with a dose reduction if you are having an active time.
