I am Amy, T1 since age 7. I reside in Ohio with my youngest son, Andrew, our cat Tuscarawas, and my Diabetic Alert Service Dog, Jazzy.

Jazzy came to me after quite a bit of advocating. I requested grant funding through Opportunities for Ohioans with Disabilities to obtain a Diabetic Alert Service Dog as part of my plan to obtain and maintain employment. I was forced onto SSDI from my career in Respiratory therapy after a stroke that resulted in autonomic neuropathy leading to severe hypoglycemic unawareness. I have since been working to regain the ability to drive and regain employment. After nearly three years of advocating, OOD approved my request for grant funding to obtain a Diabetic Alert Service Dog. Throughout the process, it was discovered that my case set not only state in Ohio but also national precedent as this was the first Diabetic Alert Service Dog funded for someone by any state vocational rehab program.

Jazzy was placed with me Oct 15, 2024 and has been life changing! I am presently looking for work and working with Jazzy so I can restart to drive!
