Question from Dublin, Ireland:
I am 24 years old, have had type�1 diabetes for 10 years, and I am currently on a sliding scale system of three Actrapid injections before meals and Insulatard at bedtime. If my blood is high an hour or so after breakfast (12.6 mmol/L [226.8 mg/dl]), can I take my lunch time insulin an hour earlier and wait an hour to eat lunch or should I stick to the regular pattern?

In general, the concept of flexible timing of injecting Regular insulin before meals based upon the premeal reading holds true. We think this is the ‘regular pattern’, and this timing might be as long as an hour if blood sugar level is high enough. It is also safer to check blood sugar over this period if the interval is to be longer than an hour.
