Question from Israel:

Our three and a half year old daughter, diagnosed with type�1 diabetes at 12 months, is currently on a regimen of NPH and Humalog at breakfast, and Ultralente and Humalog at dinnertime. We’ve noticed that she seems to need more Humalog in the morning (to cover similar amounts of carb intake) than in the evening. What factors could be contributing to this?

This is very common, and is probably related to counterregulatory hormones that cause some insulin resistance during that part of the day.

Additional comments from Dr. John Schulga:

It could be that she is starting off with a higher reading in the morning, and so needs more insulin to bring her down. She may be less active, or it may be related to the site of her injection.
