My three year old son (diagnosed at age 15 months) was originally on NPH and Humalog and has since gone on Lantus (insulin glargine) and Humalog. He is given his Lantus shot in the morning instead of at bedtime due to severe lows. How exactly is a “fasting sugar” done? How are fasting sugars done when insulin is still in the body?

Fasting refers to relationship with food, not insulin. Therefore, it is commonly understood that ‘fasting glucose’ refers to the level on rising in the morning.
