What kind of insulin regimen do you advise for our six year old son so that the times and quantity of his meals are as flexible as possible?

Answer:

For the most part, this goal is achieved either with multiple injections using pens or the more flexible insulin pumps. There are new approaches to the kinds of insulin used, especially Lantus (insulin glargine) for basal needs with short-acting insulins like Humalog or NovoLog given at meal times in proportion to the ‘carbs’ consumed. All these means can be very effective in maintaining near normal blood sugars and greatly improving quality of life and — last but not least — diminishing long term complications.

You are asking “how long is a piece of string?” Insulin regimens should be tailored to the individual and I don’t know enough about your son and his lifestyle to make this judgement. The only dogmatic thing that should be said about insulin regimens is that you shouldn’t be dogmatic about them.
