Daily Care

My brother was just diagnosed with LADA and feels that he cannot communicate at all with his doctor who is not a diabetes specialist and has almost no exposure to nor information about LADA. My brother is very wary of the level of care he is receiving and would like to be able to be treated by someone who has considerable knowledge.

I wake up around 5:00 am with high sugar levels. I am on a pump now, but in my NPH days, I did experience the dawn syndrome. I don’t think my sudden out of nowhere spike in glucose level is the result of my traveling or of a rebound. I am desperately trying to get off the pump and am afraid that with this new sudden problem, I won’t be able to. Do you have any idea what it might be?

Our diabetes team recently switched my three year old daughter to Lantus with Humalog, and but she is going low four hours after her Humalog shots, which I think this is from the Lantus. Our three and a half year old daughter, diagnosed with type 1 at 12 months, is currently on a regimen of NPH and Humalog at breakfast, and Ultralente and Humalog at dinnertime. We’ve noticed that she seems to need more Humalog in the morning (to cover similar amounts of carb intake) than in the evening. What factors could be contributing to this?

My nine year old daughter, diagnosed seven months ago, is now out of the honeymoon phase and her daytime numbers are generally good with an A1c of 6.8%, but we have noticed that the closer she gets to her 2:30 am goal, the higher she is in the morning. Is it normal for the dawn phenomenon to be more dramatic the lower the blood sugar goes? What changes would you suggest?

I have had type 2 diabetes for 16 years, I am careful with my diet and exercise, I am on Amaryl once a day, and my blood sugars are normal most times three hours after eating, but my fasting blood sugars are slightly elevated. Why? What can I do so that I don’t have this rise in blood sugars in the morning?

About 15 months ago, my 14 year old daughter switched from Toronto (Regular) insulin to Humalog. Since then, she has required over 100 units of insulin per day, and some days she is over 30 mmol/L [540 mg/dl]. She has been hospitalized 20 times this year for dehydration, and poor sugar control. Should I put her back on the “R”?
