A senior citizen with type 2 diabetes has Intense itching in the underarms, groin, under the breasts, at the hairline, and behind ears, but no rash whatsoever. Is this due to the sugar in the perspiration being released in these areas?

You may want to consider a fungal infection of skin in these areas. This is not uncommon in patients with diabetes where the area in the skin folds remains damp all the time. This can be diagnosed by your physician and treated with topical medication.
