Do acanthosis nigricans and type 1 diabetes occur together? I have a ten year old girl with autoimmune adrenal insufficiency, vitiligo, positive GAD antibodies with a low first phase insulin response (less than 1%) and has just been diagnosed with acanthosis nigricans by a dermatologist. She is thin and seems to be developing puberty normally. How can her insulin levels be very low and she have this skin problem?

Acanthosis nigricans can occur for a variety of reasons, although the most common is insulin resistance associated with obesity. You should have your endocrinologist talk to the dermatologist and together they can decide what else, if anything, needs to be checked.
