My 13 year old son takes Humalog with NPH at breakfast and at dinner. If in the future he switched from NPH to Lantus, do you estimate less Humalog for meals because the Lantus and Humalog will overlap whereas Humalog and NPH do not?

This is one form of basal/bolus regimen. The Lantus acts as basal, and its dose is adjusted primarily according to the fasting blood glucose levels. Humalog boluses are individualized and based on two- hour postprandial blood sugars.
