Question from Massachussetts, USA:
What would a two hour post meal blood glucose target number for a 11 year old active boy be? His control has been very good, in general, with A1cs in the 6.0 to 7.0 range. He uses a pump and has had type 1 for over five years.
Answer:
If a pump bolus dose is properly calculated, blood sugars two hours after meals should be approaching the pre-meal values. Remember to try and bolus at the time of the meal or very shortly prior to the meal when possible, since this will help to minimize the after meal spike in blood sugars. The spike in blood sugars will be greater when you wait to bolus until after the meal.

[Editor’s comment: Dr. Brown’s answer implies that the child’s pre-meal blood sugar is within range. The actual blood glucose reading should be within the child’s target range, which he/you should know from visits with his diabetes doctor. If the pre-meal blood sugar is high and the child needs a correction dose, it may be more difficult to ensure that the post-meal reading is within range.
