The Quilt for Life Panel Return Form will return in May of 2025, before our Friends for Life Orlando 2025 conference. Any quilt panels not claimed before the end of our FFL Orlando 2025 conference will be donated or disposed of.

Quilt for Life Panel Return Form

CWD's Quilt for Life showcased life with type 1 diabetes. Birthdate. Diagnosis date. Pumping date. 3 am blood sugar checks. Wishing for a cure. Smiles. Lots of smiles. All are part of the Quilt for Life.

We first showed the Quilt for Life at the Rose Bowl during our Pasadena 2002 conference. We had it on the National Mall in 2006 and 2008. We last showed it for the last time at our Friends for Life Orlando 2019 conference. It's time for the quilt panels to be retired.

Quilt panels were returned to many people immediately after Friends for Life Orlando 2024. Hundreds, however, remain.

Quilt panel owners who have not already received their quilt panel or instructed CWD to dispose of it may request to:

Pick up their panel at Friends for Life Orlando 2025
Have a friend pick up their panel for them at Friends for Life Orlando 2025
Pay to have CWD mail it back to them (US addresses only)
Permit CWD to donate or dispose of the quilt (panels with any attachments cannot be donated)

IMPORTANT: All quilt panels not claimed will either be donated to a charity in Orlando, Florida, or disposed of immediately after Friends for Life Orlando 2025 in July 2025.

The CWD Quilt for Life will be displayed in an online gallery.

Shipping of Quilt Square via USPS

Quilt panels can be shipped to US addresses only. Shipping is via USPS. The cost to ship a quilt square is $30. Your payment will be processed by Stripe and you will receive a receipt via email. This is considered a tax-deductible donation to CWD and you will receive a tax receipt in 2025.
