I am the school nurse (newly hired) for a medium school system. I have a 14 year old boy with diabetes student who is fairly well controlled with insulin twice a day. He is (according to his physician) spilling ketones and this is making him lethargic and unable to do his school work. What would cause this and how can I help him?

My son is 10 years old and was diagnosed with diabetes about a year ago. Recently, he went from 7.1% to 8.9% hemoglobin A1c.It seems like it doesn’t matter how hard I try to manage his diabetes, his levels are never consistent. My 11 year old stepson was recently diagnosed with type 1 diabetes and has has ADDHA. He suddenly has no impulse control and is having some type of episodes where he is not aware of his behavior. He is within his blood sugar ranges, but all those carbs turn to sugar and he has never been able to handle sugar. We need a specialist, someone who has knowledge of how these two conditions interact.

Our three year old grandson had a ketone smell on his breath, and when his blood sugar was tested by his grandfather, who has diabetes, it showed 16.3 mmol/L [293 mg/dl](4 to 8 mmol/L [72 to 144 mg/dl] normal). Five hours later at the hospital his blood sugar was 4 mmol/L (72 mg/dl). Does diabetes develop slowly in a child? Could his pancreas be working at times and then not?
